INSTRUCTIONS FOR PREPARATION OF RESEARCH GRANT APPLICATION
KIDNEY FOUNDATION OF NORTHWEST OHIO

Funding periods 2004-2005
I Period of support: April 1, 2004 March 31, 2005
II Date for submission: Applications must be received in the office of the Kidney Foundation of Northwest Ohio by 5:00 p.m., November 30, 2003. Announcements of awards will be made in March, 2004.
III Level of funding: The total amount available for research in 2004-2005 is approximately $7,500. The number of projects supported and the level of support will depend on the quality of the proposals received and the levels of support requested. Investigators are urged to make their budgets realistic.
IV Limitations on support: This granting mechanism is intended primarily for the support of laboratory investigations to include supplies, small items of equipment, and salary and fringe benefits for technical personnel. Travel and publication costs will not ordinarily be supported. Institutional overhead is not allowable. Except in unusual circumstances, approval will not be given for purchase of capital equipment with a value greater than $1,000.
V Areas of investigation to be supported: Research projects to be supported under this grant mechanism are to be investigator oriented and thus will reflect the interest of the investigator. The Kidney Foundation of Northwest Ohio is particularly interested in supporting investigators with clear relevance to human renal disease.
VI Review process: Applications received will be reviewed by a panel of clinicians and scientists especially chosen by the Kidney Foundation of Northwest Ohio to serve in this capacity. Applications will be evaluated on the basis of: 1) the potential for meaningful and timely results of the investigations proposed; 2) the quality of the background and references given in support of the project; 3) the appropriateness of the methods; 4) the originality and significance of the project; 5) the clarity and brevity of the application; 6) the appropriateness of the budget; 7) the qualifications, experience, and support base of the investigator.
VII Year-end report: Within three months of the expiration or termination of any investigational support by the Kidney Foundation of Northwest Ohio, a brief report of the results accomplished must be submitted to the Foundation, accompanied by a balanced budget statement from the institutional official responsible for grants administration.
VIII   Application form and instructions: Application forms are provided for investigators to use in preparing an application for support. Applicants are urged to make their proposals clear and succinct. Submit pages 1-3 completed on the forms provided. Page 4 and subsequent pages should be typed on plain bond paper.

I Check whether this is a new application or a renewal.
II Give a brief title of the project.
III Dates of proposed project period for this grant year will be April 1, 2004 through March 31, 2005, or some later dates at the request of the applicant, and with approval of the Kidney Foundation Research Committee.
IV Principal investigator’s name, title, department, work address and telephone and, if different, mailing address, signature. Attach a curriculum vitae.
V Give name of the institution where the research will be carried out.
VI Give the name, title and address of the official signing for the institution, who will have responsibility for the administration of the grant. Have the official sign the document.
VII Research involving human subjects: The DHEW policy on protection of human subjects (CDHEW Publication No. NIH72-102) requires that the grantee institution provide with each application involving human subjects as certification that the application has been or will be reviewed in accordance with the terms of an assurance of compliance with this policy, formerly submitted to and accepted by the Department of Health, Education and Welfare. If the project has already been reviewed and approved by the institutional committee, check the appropriate space and insert the date of committee review. This entry, on the official signed application, constitutes a certification that the institutional committee reviewed and approved the procedures involving human subjects as provided for in the institutional assurance. If the project involves human subjects, but has not yet been approved by the institutional committee, check the appropriate box and insert the date the project is scheduled for review. If circumstances are such that it is impossible to submit this certification with the application, it should be forwarded as soon as possible thereafter.

VIII Give the names, titles and institutional affiliations of other faculty level personnel who will be collaborating on this investigation. (Attach a CV for each collaborator.)
IX List other grants, with amounts of support, of the principal investigator which are either currently in effect, approved, or planned. Indicate beginning and terminating dates for each, plus dollar amounts.

X Prepare a realistic budget.

For personnel, give names when possible, their position, the percentage of time which will be spent on the project for which support is requested, a dollar amount for salary and fringe.

Itemize any equipment for which funds are requested, keeping in mind that special permission will be needed for purchase of capital items with a value greater than $1,000. For supplies, itemize those major areas of expense. Funding will not be approved for faculty level salary support, travel, or publication.

XI Description of research plan:
 
A. Introduction
 
1. Specific Aims (objectives of the proposed research)
2. Background (previous work by others and current status of research in this field; give references)
3. Previous work by applicant (related to the proposed research; give references)
4. Rationale (present reasoning behind your approach to the problem)
B. Methods of investigation: Detail the methods, procedures, techniques to be used and the means of analysis or interpretation.
C. Facilities available: Specify clinical or laboratory facilities, major items of equipment.
D. Significance of proposed research: Potential importance or contributions...in particular, health related implications.
E. Summary in lay terms of proposed investigations and significance: Provide a short (less than one page) clearly written summary in lay terminology of your project, its aim and significance.
NOTE: Keep the proposal as clear and succinct as possible, remembering that the reviewers will have a number of proposals to read, and those which are clearest and most realistic will undoubtedly make the most favorable impression.

Remember to include with the application curriculum vitae of the principal investigator and collaborating investigators, if any.

Send five copies of the completed application to Research Review, Kidney Foundation of Northwest Ohio, 1545-B Holland Road, Maumee, OH 43537. Applications are to be signed on page 1 by principal investigator and institutional official named in Item VI.
