your name and mailing address. Also tell us the name of the person
who will receive the certificate that indicates a gift to the Kidney
Foundation of Northwest Ohio was made on their behalf.

the certificate, you will receive lollipops to put on your Christmas
Tree. You will also receive the satisfaction of having helped make
life better for the nearly 5,000 people in northwest Ohio diagnosed
